Аннотация

По данным современных когортных исследований, болезнь Шегрена дебютирует в детском возрасте примерно в 1% случаев, однако не менее 5% взрослых пациентов с болезнью Шегрена отмечают появление первых симптомов в возрасте младше 12 лет, что указывает на недостаточную диагностику данной патологии у детей, которой способствуют не только существенные отличия в клинической картине заболевания у детей и взрослых пациентов, но и несовершенство диагностических критериев, а также недостаточная осведомленность врачей о болезни Шегрена у детей, при том что сведения о симптоматике, подходах к диагностике и лечению этого заболевания у детей в русскоязычной научной литературе практически отсутствуют.В первой части лекции представлены данные об эпидемиологии, этиологии и патогенезе болезни Шегрена, особенностях ее клинической картины у детей, включаяксерофтальмию, ксеростомию,паротитиприпухлостьподчелюстнойслюннойжелезы, поражение других экзокринныхжелез, мышечно-скелетные, почечные, легочные, нейропсихические,кожные, желудочно-кишечные, гематологическиепроявления заболевания, егоконституциональные симптомы,объективныеисследованиясухости, лабораторные, визуализирующие, функциональные, гистологические исследования, а также обсуждаются подходы к дифференциальной диагностике болезни Шегрена у детей.
