Факторы риска развития и неблагоприятного исхода респираторного дистресса у новорожденных

Аннотация
Респираторный дистресс — наиболее частая причина критических состояний у новорожденных, требующих госпитализации в отделение интенсивной терапии, и сопряжен с высокой заболеваемостью и смертностью. В обзоре продемонстрировано влияние различных анте-, интра- и постнатальных факторов на течение и исходы респираторного дистресса у новорожденных. Поиск статей осуществляли в реферативных базах данных Pub Med и eLibrary.ru, с января 2015 г. по октябрь 2025 г. Использовали ключевые слова: «respiratory distress», «newborns», «preterm infants», «risk factors», «респираторный дистресс», «новорожденные», «недоношенные», «факторы риска». В анализ вошло 50 публикаций. Наиболее значимыми факторами, влияющими на исход, были возраст матери более 35 лет, прием матерью антидепрессантов во время беременности, наличие у матери артериальной гипертензии, отсутствие антенатальной профилактики кортикостероидами, необходимость проведения реанимационных мероприятий в родильном зале, срок гестации менее 28 недель, мужской пол новорожденного, наличие апноэ, гипотермии, респираторный дистресс-синдром новорожденных, малая масса тела к сроку гестации, дефицит витамина D в пуповинной крови. Пневмония, асфиксия и ранний неонатальный сепсис являются основными причинами острого респираторного дистресс-синдрома различной степени тяжести. Основными заболеваниями у недоношенных с экстремально низкой массой тела были респираторный дистресс-синдром новорожденных, бронхолегочная дисплазия, ретинопатия недоношенных, некротический энтероколит и внутрижелудочковые кровоизлияния различной степени тяжести.Отечественные исследования, посвященные оценке факторов риска неблагоприятного течения респираторного дистресса у новорожденных, единичны, а работы зарубежных авторов крайне противоречивы.В то же время поиск высокочувствительных и специфичных предикторов неблагоприятного течения респираторного дистресса в неонатальном периоде позволил бы значительно улучшить исходы лечения.
