Микробиота кишечника доношенныхи недоношенных новорожденных: рольв развитии инфекционных заболеванийи возможности коррекции (обзор литературы)

Аннотация
Введение.Изучение микробиоты кишечника новорожденных является важным направлением современной медицины, так как ее становление влияет на иммунитет, метаболизм и здоровье детей. Особенно актуальна эта проблема для недоношенных детей, у которых замедлено развитие микробиоты, что повышает риск инфекций и хронических заболеваний. На состав и функциональное состояние микробиоты существенно влияют способ родоразрешения, антибиотикотерапия и тип вскармливания, хотя это и требует дальнейшего изучения.Цель—систематизировать современные данные о факторах, влияющих на микробиоту кишечника доношенных и недоношенных новорожденных, и методах ее коррекции.Материалы и методы.В статье проведен обзор научной литературы, включая клинические исследования и систематические обзоры. Использованы данные о методах изучения микробиоты, влиянии на микробиоту способа родоразрешения, антибиотикотерапии, грудного вскармливания и искусственных питательных смесей, лечения пребиотиками и пробиотиками.Результаты.Здоровая кишечная микробиота новорожденных играет значительную роль в профилактике инфекционных заболеваний. Нарушение микробиоты кишечника, вызванное кесаревым сечением, антибиотикотерапией и искусственным вскармливанием, сопровождается снижением колонизационной резистентности и ростом условно-патогенных бактерий (Klebsiellaspp.,Enterococcusspp.,Pseudomonasspp.), что повышает риск развития сепсиса и некротизирующего энтероколита. Применение пробиотиков и пребиотиков способствует снижению инфекционной заболеваемости за счет восстановления микробного баланса, укрепления барьерной функции кишечника, что делает коррекцию микробиоты важным направлением профилактики и терапии инфекций у новорожденных.Заключение.Формирование здоровой микробиоты у новорожденных критически важно для их долгосрочного здоровья. Современные данные подтверждают значимость естественных родов, грудного вскармливания и рационального использования антибиотиков при выхаживании недоношенных детей. Пробиотики представляют перспективное направление для коррекции нарушений микробиоты новорожденных, однако необходимы дополнительные исследования для оптимизации их применения.
